We travel to assisted-living communities, adult foster homes, residential-care facilities, and other senior living environments.

Rehab & Skilled-Nursing Facilities

A mobile appointment can be arranged at rehabilitation centers, skilled-nursing facilities, and transitional-care locations.

Private Homes

Urgent notarization may also be completed at a private residence when a person is unable to travel because of illness, limited mobility, recovery, or end-of-life care.

Remote Online Notarization (RON)

A remote online appointment may be available for eligible documents and signers who can complete the required identity and technology steps.

How a Hospital or Rush Appointment Works

Request the Appointment

Call Fortis or submit the Hospital & Rush Notary Request form with the signer, document, location, and timing information.

Review & Confirm the Details

We review the location, identification, documents, requested timing, facility access, and expected charges before confirming the appointment.

Prepare the Signer and Documents

Have the identification, documents, witnesses, facility instructions, and assistive devices ready. Leave required signatures unsigned.

Meet at the Location

The notary checks in, meets the signer, verifies identification, reviews the notarial certificate, and communicates directly with the signer.

Complete the Notarization

When all requirements are met, the signer completes or acknowledges the signature and the notary completes the journal and certificate.

Hospital and Rush Notary Pricing

Oregon regulates the maximum amount a notary may charge for the notarial act itself. Oregon currently permits a fee of up to $10 for each traditional in-person notarial act and up to $25 for a remote online notarization.
